Centralized Intake

Polk County offers a centralized intake approach through community partner Primary Health Care. Centralized Intake offers a coordinated approach to addressing the needs of individuals and families experiencing homelessness. While completing an intake, clients complete an assessment to assist individuals and families in resolving their homelessness and may be assigned to a case manager, referred to shelter, or referred to other agencies for further assistance.
