Youth Homeless Planning

The Polk County Continuum of Care Board is pleased to present Ending Youth Homelessness: A Plan to Ensure Housing Safety and Stability for Youth and Young Adults Experiencing Homelessness in Polk County. This plan represents months’ worth of discussion and ideas, data analysis, and examination of best practices from a wide range of stakeholders with commitment and guidance from youth, direct service providers, funders, and policy makers.  The plan is organized under four priorities; Safe, Strong and Stable Families, Early Intervention, Pathways out of Homelessness, and Community-Level Solutions to System Challenges.

The planning process was generously funded by the Mid-Iowa Health Foundation and led by Iowa Homeless Youth Centers, the Polk County Continuum of Care Board, and the Youth Policy Institute of Iowa. The newly formed Youth Work Team, under the Polk County CoCB, will guide the implementation of the plan. This team will be responsible for maintaining momentum for the plan’s strategies, tracking and communicating progress, and convening stakeholders as needed.  See the plan under our Youth Homeless Planning tab.
